1. Receive your Personal Identification Number (PIN)
When you come to Mid-Valley Medical Group for an appointment you will
be provided a one-time Personal Identification Number (PIN) enabling you
to gain access to our secure online tools. All patients are required to
use this unique PIN to verify their identiy. You will also need to have
a unique email address on file with us. If you have not provided one, please
stop by the clinic before attempting to register. PINs are good for 30
days from being issued. If you have not received a PIN letter or if your
PIN has expired, please contact the clinic to have one generated for you.
If you are attempting to register for a child or family member, please
contact the Portal administrator. This option should be set up in January
2015. Instructions will be posted here when avaiable.
